SLB Family Nutrition is a sole trader, registered with and regulated by the Health and Care Professions Council (HCPC), registered office; Health and Care Professions Council, Park House, 184-186 Kennington Park Road, London, SE11 4BU. Registration number DT17606. I aim to use my scientific knowledge and expertise as a dietitian in an accurate and professionally responsible manner.

I never make or support unjustifiable statements relating to particular products (as per HCPC & BDA guidelines) or promote one product over another (as per food industry regulations).
All of the opinions on this website are my own and are 100% honest and I will highlight if I have accepted money or samples in the process of formulating those views.
Whilst every effort is made to ensure that the information detailed within this website is believed to be true and accurate at the time of writing, we cannot provide any warranty or guarantee for the accuracy, completeness, suitability or timeliness of any material found on this website for a particular purpose.

Terms and conditions for Consultations

Sarah Lindsay Brown offers dietetic consultations.
A feeding assessment (not treatment) may lead to referral to other healthcare professionals.
Once you have booked a feeding assessment you will be asked to fill out an assessment form, provide a food diary using an app (we are unable to analyse a food diary using other formats) and send a video of your child’s mealtime (this is optional).
After initial assessment you may be invited to either a package of consultations (if not referred on). Or an initial consultation and follow-up.
Consultations are conducted via zoom.
Cancellations:

Group bookings are non-refundable.

You will be charged at the full consultation fee if you do not attend your appointment or if you cancel your appointment with less than 48 hours notice.

If you cancel before 48 hours you will get a 50% refund.

You will still be charged for your full appointment time if you arrive late. The appointment will run till the end of the scheduled time. Consultations cannot overrun due to late arrivals.
Refunds:
If your child’s condition has not improved after consultations, this requires further investigation and further appointments can be arranged or a referral to another healthcare professional may be necessary. If you have consented details will be passed on to other healthcare professionals. Refund requests cannot be honoured.
In between appointments:
For 1:1 consultations, two email enquiries in between appointments are included in the consultation fee; the purpose of these is to clarify advice, not for further assessment or advice.
Subsequent advice requests via email will require a further consultation.
I cannot chase for bookings of follow-up appointments. It is your responsibility to book these in with me. I will suggest a suitable amount of time between appointments.
Payments:
Payments are to be made in advance of booking.
Consent:
You are consenting to receive advice by booking a consultation.
At the time of booking you will have the option to join an email newsletter.
Under GDPR, your details will be held. Further information on how we use your information can be found in the Privacy Policy. You can change your preferences at any time by writing to us at sarah@slbfamilynutrition.co.uk
If you no longer wish to hear from us, please contact sarah@slbfamilynutrition.co.uk to unsubscribe from communications, confirming your name and address and stating that you wish to unsubscribe.

Declaration:
By booking a Consultation, it is implied that you agree to these terms and conditions and consent to the dietetic assessment and advice given by a Registered Dietitian.
