This privacy policy, as part of the consent form, sets out how SLB Family Nutrition uses and protects any personal information that you provide us. SLB Family Nutrition is committed to ensuring your privacy is protected and complies with the General Data Protection Regulation 2018 (GDPR) and the Data Protection Act 2018.  Should we ask you to provide certain information by which you can be identified when using our services, you can be assured that it will only be used in accordance with this privacy policy.

You directly provide SLB Family Nutrition with most of the data we collect. We collect and process data when you:

  • Register online or place an order for any of our services

  • When completing our contact form, assessment or feeding assessment form or provide any feedback to us or via email

  • Use or view our website via your browser’s cookies

  • To respond to enquiries when completing our contact form

  • Professional clinical record keeping of client information

  • Sharing information with relevant health professionals

How long will information be kept?

Child records will be kept until the child’s 25th birthday as detailed in Records Management Code of Practice for Health and Social Care 2021. For adults data will be kept for 8 years.

 

How you can withdraw and request to be deleted from our files
